My intrusive thoughts usually come about due to low self-esteem and depression. Keeping busy works until the depression kicks in and I'm unable to get out of bed. Rational thinking "That's not right" "I feel that way but it's not true because" goes a long way to prevent a tragedy, but it doesn't stop the thoughts or feelings.
