Fiction is separate from reality, an 11 year old should be able to tell the difference between a plot device in a story and psychotic irl abuse, and if they can't tell the difference they shouldn't be reading 18+ content (tbf they shouldn't be reading this stuff at their age in the first place but that's really their parent's decision in the end).
