I'd need more dialogue for context, but I'm pretty sure the the top line's purpose isn't actually to necessarily convey meaning through the specific words themselves but rather to function as an interjection, which makes sense considering the second character, 哇 ("wow"), and the line beneath it, 好痛 ("that hurts").
