Isekai Yakkyoku
I love this! Update: My opinion remains the same. Mysterious is the MC's identity in the other world. Love how he is determined to be the best "pharmacist," rather than the medicine god, as in him focusing on modern treatments and not totally relying on his god-like abilities. There's typical "H" moments where he's surrounded by big breasted women when he's like eleven years old (which is problematic but let's not talk about that). There is a bit of a "harem" vibe, but the story content overshadows all of that. Satisfied me in terms of medical knowledge, albeit its cliché-ness. Also has some surprisingly emotional moments of character development. Want more!

Review: The plot twist had me shaking in my boots. The interpretable ending... I see it that the MC tried to kill his stalker/lover. He attempted to ask him a question. Depending on his answer, he would act/not act on it. That’s why he’s hiding the knife in the last panel. I would’ve thought he would kill him the moment he saw his mother’s dead body, but it seems that they’re both mentally unstable to some extent. A match made in hell, I guess.

Review: Gosh, I need time to process this. You can't even call this a romance. There are so many parallels, and the story itself is very complex. If you wanted to know about this story more, you'd have to read it yourself. It was certainly an experience that cannot be rivaled by any other work. Their dynamic was... cold master x weak puppy? But the puppy was actually a dog that wanted to control the master? But in the end, the dog couldn't and ended up just as weak as when he was as a puppy.
