If the future Noha and Suwa didn't have a kid, I think I would be 100% for Noha/Kakeru, but they did and even with the parallel worlds, its sounds wrong to me for a child not to be born because Noha choose someone else. Kakeru to keep on living is probably the number one priority and I feel that they should just go for an ot3, Noha/Suwa/Kakeru and still have the child, a happy ending no? No matter what direction they choose though, this is a lovely manga with beautiful artwork and deep understanding of sadness, that it doesn't just go away and it the author knows that love rivalry is not the way but friendship is.
