Hana wo Tadoreba Kimi no Yubisaki
Starts with the Seme’s ancestor (look alike) who was in love with a male priest (?) since there was a mention of a divine shrine where they met. The man fell ill, and the priest made a deal against fate and god’s will to extend his life, in exchange for his own life and being further punished when the shrine was struck and burned down by lightning. The man fell in despair and tried to cope with the loss by painting the priest in a scroll, which came to life and took the original’s name and appearance. The man died and the scroll was passed down to members of his family, until the MC appears a few generations later. Basically it’s not a reincarnation story. The original lovers have both passed, the MC is the descendant and the uke is the image/memory/love from the scroll that came to life.
