A lotnof people have been asking about Ci Nian's 'deformity'. Reading the following is a pretty good explanation of what might be the case with him.
I'll explain a few terms in a way that can help people understand it better (The definitions might not be correct, but I promise they should help)
Chromosome: They basically decide your appearance. You have a certain eye color due to your chromosomes.
XX - they are chromosomes. A person who has XX chromosomes is female. People who are male have XY chromosomes, but we don't need to know that here.
Uterus, fallopian tubes, labia - these are parts of the female reproductive systems, that means ONLY people with XX chromosomes have this.
hormones - These are secretions which cause a person to look more like a specific gender. Male hormones will cause a person to look more like a boy, they might get a deeper voice, have facial hair etc. A trans man might be taking these.
fetus - a baby when it's still developing
46, XX INTERSEX
The person has the chromosomes of a woman, the ovaries of a woman, but external (outside) genitals that appear male. This most often is the result of a female fetus having been exposed to excess male hormones before birth. The labia ("lips" or folds of skin of the external female genitals) fuse, and the clitoris enlarges to appear like a penis. In most cases, this person has a normal uterus and fallopian tubes

Yeah, but it's super rare. I haven't heard many case about this but I've heard about a few cases which go the other way around: A person who has testicles didn't grow a penis.
