I "think" the mc is no longer inlove with the shithead, he believes that he is, maybe because he's his only friend and the person he grew up with, maybe along the way he fell out of love and it just became an obligation or habit to care for the shithead which prob made him feel or think that he is still inlove not until ML came which is "maybe" slowly making him realize that "hey, I'm not inlove with the shithead" (I'm just yapping some random stuff so there may be a lot of grammatical errors but yeah ╥﹏╥)
