This is just my observation but a lot of shoujo manga only proceeds with doing couple stuff like holding hands, hugging and kissing after the FL loses weight. This has happened enough time for me to recognize it is pattern among these works. I don't even wanna dig deeper to know the reason behind this. (╬￣皿￣)

All though it described them as doctors it was not specified exactly their positions or when exactly they started working. So the earning was probably not enough or maybe it was but at some point in the beginning they didn’t have money since the grandma had to take care of the children so them working everyday none stop meant they wanted to earn enough money for their kids future education, being as she didn’t have no problems entering in a good prestigious school. Not saying children should face this but it’s because of this reason she learned to self adapt and enjoy many skill.. maybe at some point the parents could afford to hire someone but maybe she didn’t feel comfortable …idk I like to imagine background stories of the What if lol
