I still think it's fake. It's too suspicious that 1)Elizabeth never mentioned a brother. 2)No photos/flashbacks of the family have 2 kids. 3)Sebastian is supposed to know everything, but even he's shocked.

So I'm thinking 1)Undertaker created an artificial body/doll. Who's in it? Vincent? 2)Another demon. Sebastian is not in his true form, it's just one he decided to take. 3)Some kind of weird-ass experiment happened to Ciel before he made the contract that separated him into two people. 4)Another survivor from the imprisonment that Ciel forgot & abandoned after the contract got plastic surgery to look like him. They did have early body modification surgery back in the 1800s.

1) What does Elizabeth's brother got to do with this?
2) It's understandable that there are chapters that doesn't show having 2 kids at the Phantomhive Family but there are hints just that you didn't noticed. Also I won't argue on the Vincent part since it has become a theory now.
3) That's still a mystery but all we know he and the other kids were sacrificial lambs and not experiment so its still mysterious.
4) Sebastian said that there are 'no' survivors so when Ciel asked to Sebastian if he was lying to him, meant if Sebastian was lying to him that everyone died that day and Sebastian said he doesn't lie, means NO one survive that fire.
5) But there is a big possibility: UNDERTAKER HAS GOT TO DO WITH THIS AND THAT'S MAKING OUR CIEL SUFFER DD:

1)Elizabeth never mentioned Ciel having a brother. Also, Her brother also never mentioned Ciel ever having a brother.
4)Thank you for clearing that up. I'd forgotren that.
5)I think no matter what, Undertaker is behind it.

1) Perhaps Yana-sensei just wanted to confuse us XD She is a great plot twist writer but she did give a hint when Elizabeth noticed that our!Ciel was shorter and he only disappeared for one month, Elizabeth couldn't grow so fast in one month and real!Ciel who was engaged to her is taller or same height as her. But still Yana-sensei sure is a plot twister master XD ao we might wait for next chapter. The question still left unsolved. Oh yeah, he never did. Yana-sensei got something on her sleeve thinking again xD
4) It's okay, Just helping out and I think the weird experiment will go to real!Ciel whose existence still unknown to us
5) Yes, Undertaker is the final boss in Kuroshitsuji or Yana-sensei will do some plot twisting again xD

It's not really a cleaver twist. And if we are following Knox's rules (which is seen in Book of Murder) we were not "duly prepared for them", (and yes I know that supernatural agencies are not allowed, but no supernatural entities are ever the main culprit so...).
