Essentially the many eye dude turned on Cain I think. Cain was trying to get the doctor to take the bullet out of his chest, so he sent the many eye dude and gave him a compass that helps find things. But the eye dude used it and "killed" one of the doctors duplicate bodies. So the doctor went to find Cain himself to ask why he was sent to be killed and found the eye dude aswell. So the doctor was trying to see if Cain knew the If the eye dude tried to kill him or what was going on. Cain figured out by the doctors dialogue that the eye dude had betrayed him by trying to kill the doctor. So the eye dude tries to kill the doctor once again before it's too late and Cain tries to stop him. I think that's it sorry it's really long
