my friend, that quite literally is the trope they went for and is a key part of the plot. reread chapter 8 and 9
he has to be cleansed by an alpha in order to live, it’s just a matter of time when his body will completely break down. the effects of all the suppressants, drugs, and eye drops has damaged him beyond the point of no return and the effects probably multiplied after his mom forced him to take the full dose again in one of the latest chapters. like he’s heavily poisoned and his only chance to live is to have sex (hence the fuck or die trope)
correct me if im wrong, but also i think the mom also said she’ll find an alpha to cleanse him and then kill the alpha afterwards :3 so contact with another alpha is necessary to the plot and him fainting is probably his body (slowly) getting even weaker

imo the start of their relationship is happening right now but they aren’t able to confess yet, as one of them has yet to realize his feelings. im guessing u mean u hope they don’t start their relationship only because of the sex, however there was already tension between them (emotional and physical) so it wouldn’t be baseless. although the prince is unaware of his feelings for now, rishar is one the very few people he can trust and would be vulnerable with. like i get what you’re saying, but they’ve already built a relationship that’s so full of love, respect, and trust so sex being a catalyst for them (to get together) wouldn’t be sad
the prince and rishar care for each other deeply and would do what it takes to make the other person safe/happy. with that being said, rishar wouldn’t have sex with him without consent or do it for his own feelings- he values and honors the prince’s life so them having sex is something that (im hoping) rishar leaves in the hands of the prince. rishar’s character would be completely screwed over if they made him SA the prince or if he took advantage of the prince’s illness/was self-motivated when agreeing to the treatment
