The doctor is skeptical to blurt out that Sid is betraying Cain so he's finding ways to let them both know what he knows since the doctor doesnt know if both of them want him dead or needed. The Doctor doesnt want to blurt out that Sid wants him dead since he knows Sid is Cain's right hand so he's really careful here thats why there is a long of monologue happening. Hopefully in the next chapter, we will get some action.
It seems the doctor was finally able to mention an important piece that only Cain and Sid knows. He honestly played it well.
