I think the wound got infected since it's big and all, plus they don't have a healer in their team (I think).
I think it's because the medical knowledge of that world is so primitive, that it's like living in the medieval ages where their wounds are treated poorly so it could fester easily as well.

I don't think that the mangaka treated it lightly though. after the noncon scene Yuuri was mad at Victor for the whole night- then the next morning Victor was remorseful. then when vic & yuuri tried again victor was scared of hurting yuuri. they didnt do the do until yuuri reassured him that he was willing.
