Yeah i thought that manhwa team changed his gender since the fairy king in novel also became fairy queen in manhwa. After reread, i realized he actually still male omega yay (idk about the fairy queen tho). His outfit with big puffy sleeves and lace, including his long hair really designed very well, since in novel he hold the title as the most beautiful omega in country.
at first, I wasn't sure what gender the Empress was, but his gender makes sense for his hatred towards MC.
the Empress is a male omega.
the marquis (Empress's father) & MC's physician is a male beta.
in this world alphas & omegas are looked up to while beta are discriminated against especially in this kingdom (people use the fact that the laws don't let betas ascend to the throne to justify this).

so beta marquis was finally a commoner who got his title by saving a lot of people with his medical skills (either during an epidemic or war, I don't remember) he over the years became the previous emperor's imperial physician and saved either the previous empress or consort the previous Emperor deeply loved...previous Emperor asked what marquis wanted as a reward. He requested his son (dominate omega) to marry the then crown prince----resulting in the current situation.
