Nope this is a work of fiction and things are exagerated. First of all intern are not allowed to be themself they are mainly assisting and observing most of the time the doctor. Unlike in this drama that intern are attending the patient and making decission. The doctor will explaining things to you while attending the patient its like a classroom discussion but its real time.

Its exagerated like every misstress/affair drama.
Its not a heirarchy persee but rather in real life its impossible to have an intern more knowledgable than a resident no matter how genius the intern in school since school curriculum and actual practice are so different.
