I wonder why there aren't much yaoi manga with adoption of a child... I know all these are fantasy. But id like to read a realistic setting. Where they get old but adopt children of their own..

I think because yaoi has such a strong target audience of young women that thats what they're interested in. Hot young men getting it on with each other.
A matured couple that has kids or even yoais that have just older MCs are kind few and far between.
But I think these kinds of stories are picking up as more LGTBQA+ authors, readers and issues become more vocal and apparant.

There are actually several of them (manga), where it starts out where you THINK the child belongs to one part of the couple from an earlier relationship, but come to find out that they have adopted a family member's kid, a neighbor's kid, or well...adopted the child that is already there in the story.
You won't see the theme of going through an adoption very often. It is a touchy subject in most countries, and adding a couple that is LBGT into the mix could potentially get that particular work censored or completely thrown out. Unfortunately there are MANY countries in the world who feel that a couple outside of the traditional married, male/female relationship, cannot raise a mentally healthy and sound child (and yes, that includes unwed/single parents). If you are living in a country that does allow such adoptions, consider yourself very lucky and blessed, because there are countries out there that will take a child away from a parent at the mere whiff of a "questionable moral act."
