If you actually read the manga instead of writing it off as a gross fantasy, you'd know that the portrayal of "incest" isn't what you'd expect. Slight spoiler but they don't really become physical with each other and "incest" is addressed realistically and tragically here. While yes, it is sad, it also paints a portrayal of a realistic sibling as well as "people in love" relationship and I feel like that's important to see, especially in yaoi manga that is usually full of sex kinks and tropes.
