Hosik doesn't call him Hyung tho. He calls hum Sunbae, which sets a large barrier between them. But that makes sense since Hosik isn't in love with Sungyeon (although there is budding interest) nor has any feelings of affection.

I agree with hosik calling him sunbae to put a barrier, however, there is love between the two. Hosik has been interested with sungyeon since around the time that sungyeon has been in love with him. Not all relationships have to be cutthroat yes or no. Hosik doesn't want to let himself love sungyeon, because sungyeon is a rich spoiled young master and not only that but he got pregnant in a world where men getting pregnant would be considered an abomination, especially in a society that only cares about looks. The only, ONLY reason it even seems weird is literally the art style. Somehow, the art style makes the characters look slightly off, and creates a creepy air to what is honestly a normal yaoi with mpreg.
In korea, you cant just drop formalities even if you married to that person, sunbae is more formal but at the end of latesr chapter Hosik is confidence enough to call him Hyung *more endearing terms
