I'm also confused by Kurose's comment about his love for Shirotani and I can't believe he meant that. It would be awful and unromantic, very disappointing and it would mean Kurose is insane. I don't think it was what Rihito Takarai meant (but I may be wrong).
Even if, maybe Kurose has some dark little secrets from his past (as he says he's twisted), I think he needs Shirotani's attention all for himself and he may be shameful about that. It was maybe the true meaning of his comment: he noticed Shirotani because of his mysophobia when they first met and, gradually, they began to talk to each other to cure Shirotani. Maybe, Kurose became fascinated by what he saw about Shirotani (including his disease because his mysophobia is a part of his life and a huge parameter which made Shirotani who he is now). And Kurose fell in love with Shirotani (the man with mysophobia, the man with his strength and weaknesses). Then, maybe Kurose meant he loves Shirotani because of his disease which means he loves the man Shirotani is now (with his disease).
It's more "I love you as you are" than "I live you for your disease." Folk with OCD are often dumped by people who think love will cure them. Fiction is always promising that love will cure mental illness. Love cures nothing. But if you can be loved FOR your weirdest, ugliest side... How often can I say I love this manga?
