i'm pretty sure it's because they also see themselves as fat like Osu does, and doesn't believe anyone that tells them otherwise, which is what Osu also experiences. It's basically showcasing their distorted body images of their own body.

After reading the raws there is no doubt in my mind that either the pretty boy knows the girl in the pink dress OR the girl in the pink dress is going to come back and be a plot point.
