At the beginning the uke is just one of the bodyguards ( who have to abide lot of rules how to act around him) who is a beta, because he can't remember. Although he will be aware that Josh looks suspiciously handsome for a bodyguard who pretends to be a beta.
And let's say that guy has several problems like traum, family issues, extreme alpha identity problems, needs medicine...a lot of...which have side effects . He is not like Keith, but has several problems that makes him violent and act in a crazy way.

I'm going to add, since a lot of others already write this in the comments. Their relationship dynamics at the end, it's more like the other way round. The uke is the more dominant one in their relationship ( which is rather unusal for a/b/o), so you don't have to worry about Josh getting a bad matchup, as it looks at the moment/for some time.
For the ratings, probably readers who stopped reading without finishing the story? As i already said, you see in the coming chapters how violent and crazy the ml can be, also towards the uke who is just a bodyguard for him at this point. You have to get through that. And if it's important for you, the seme has never and will never rape anyone, he is even afraid of that.
