I feel like brother was purposely dying in order to get closer to his sister. Like when she fell in love in the past and he drank the poison that was meant for her and her lover. Or when she sent him away multiple times and he always died.
One brown brother even pointed out while he was dancing with his sister that he might have fell on purpose.

After re-reading this manga (why? Because I just wanna see the world burn) I noticed that not only are they cousins (their moms are sisters) the uke is also their uncle (the ukes mom was given to the head of the family, while the sister that didnt have the mark was given to the son of the head of the family)


And considering the genetics, its a high possibility that the children born will be abnormal
