The potion he takes forcefully keeps him from growing/aging so he stays in a state of pre puberty and can pass as a girl. If he stops taking it though he begins to rapidly progress to his real age putting a lot of strain on the body and causing immense pain. Once he turns 18 he can inherit though and will gain the power to protect himself. Which is why he’s got a potion to do the opposite of the one he’s taken. Once he turns 18 and is an adult who can separate from his family he plans to take the reverse potion and once he’s suffered through the process of aging to his true age he plans to leave and take Blair with him.
