okay so from what i understood, the twins father forced the main characters brother to get surgery to turn into a women. My theory would possibly be that he wanted the brother to be turned into another gender so that their relashionship would seem normal to the outside. Clearly the brother (or now sister) never wanted the surgery and also said in the last few panels that he "never" asked for "this" and i assume he refers to his body.
