Hmm, I think the dog/wolf was a representation of the ML's instability. We constantly see his eyes in smoke/through darkness, similar to a predator watching prey. It also represents his strength and domination.
The act of the wolf mouting the MC and raping him with blood ect shows how MC's losing control in his waking life. It made sense because that first psychological session with the ML just backed the MC into a corner. He was oppressed.
The rape also signifys the ML's sexual desires towards the doctor, but also highlighting how he was nothing more than a rabid beast following instincts rather than rationality and emotional maturity. Considering how he was raised, it makes sense.
