Well if you don't hate novels then you can read The reader and protagonist have to be in true love. This doesn't have pictures however it is one of my most favorite stories of all time. If you want to read it in novel updates. Also the protagonist isn't the one who the world is against its actually he ml. (=・ω・=)

Raising a Bat maybe? It's shonen ai I think and I remember feeling terrible for the main character most of the time I was reading it. Idk if that's what you mean by "the world is against" but he definitely suffers
