As a MC I feel annoyed by his too saintly character but as a doctor it is exactly just right. The story lacks morally gray supporting character to help MC deal with threats since MC is too nice. The story probably will surround MC turning enemies into allies or use luck to deal with enemies.

I do think Mc is too nice but if you think about it, he came from a modern world and was a nice japanese doctor(they are usually very polite and committed to their jobs like mc) so his attitude can be somehow acceptable. Now I don't think the are not gray characters here for instance the apostles and the Pope are a good example. It's just that mc is being protected in the world tree so he haven't had the opportunity to face too malicious people. Let's remember he almost die 4 or 5 chapters ago xD

I like the story but the male:female ratio makes me worried that it will be another harem. As a story about a doctor spreading medicine science in a fantasy world I don't want unnecessary "female admires heroic genius MC then follows him then fall in love" trope. I hate this kind of catering fanservice to male audience rather than pure story centered to attract fanbase.

I TOOOOOTALLY get what you mean. HOWEVER! I think the ratio is fine.... Buuuuuuut.... so far I agree that the MC is getting harem-ish. Personally, I think I can give it a pass ONLY because the gender ratio of characters is fair. Also the style of art kinda lends itself to more female characters... so more females to males is going to be expected but with so many characters its not too noticeable
If anything this art style gives me an pause for the "beautiful people are right trope, with the occasional psycho beautiful person" and visa versa.

If I may add to the list, our MC clearly has no romantic intentions at this time; it's hard enough to get that man to get some sleep. And often in fantasy or medieval setting stories, female characters get coded for healing roles; there are still medical fields in the real world that are female-dominant. Men still dominate physical labor jobs and with the dragon prince and prior dwarf king making tools for him + the dragon prince playing transportation every now and then, it still runs true to reality.
If we look at the church, even, we see it more as a political ground than a medical sphere. We see more men there. I'm not saying the men aren't interested in actually healing people, because clearly, we have one that is fed up with the church corruption and will help any human. But we get a softer, comforting atmosphere with our MC and his surroundings to contrast with the harshness of the norm for medicine in that world. A bunch of women are easier to do that with and as you said, they aren't out of place yet.
And I did notice the uglier people were being the bad guys but with this being a medical manga, my hope is that we have someone with old scars or damage that cannot be fixed because of the time that passed and either becomes a regular member or becomes an apprentice, even. I don't see a plastic surgery plotting being possible but a burn victim should show up and grafts can be done.

I don't get why many people feel offended to the point of harassing the author. People complain they feel betrayed because of the BL like atmosphere without tagging but come on. It's a bromance at most without actual relationship.
It can't even be considered BL as the relationship isn't even up to the level of Naruto/Sasuke bromance which were straight.
I get betrayed by works where fantasy actions becomes romance/polygamy unnecessary to the plot midway without tagged but I don't see anyone destroying the authors for it. People are too homophobic for an ambiguous bromance but full on support for harem development for a horny MC.(〜 ̄△ ̄)〜

This is definitely anime material. I like situations when their eyes glow with stars in it during big moments, it's a unique signature of this manga and definitely a plus for me.ヾ(☆▽☆)
I like these teacher-student genre, the only thing I worry is unnecessary erotic side character for fanservice/comedic relief that exist in JP mangas/anime.
Artstyle change looks like a generic mass produce pornhwa. I like the previous art better and looks more original and doesn't look awkward.( ̄∇ ̄")
