Considering Guwon is not human and the townspeople are scared of him and others knowing about their illness, it could be that Guwon can only eats people and that in return for protecting everyone, he eats the ones that are likely to die anyway? Seems a little far-fetched, but also it might explain the vomiting scene if he ate something normal during the day and his body can't digest it?

Nah I'm just speculating - him not being human hasn't been confirmed, just author is hinting to something in here. The fact that everyone's scared of him/act really weird, he doesn't know anything including very simple things people who grew up in society with would have known through social exposure, we never see him eat or drink, and I think once MC said he felt cold (i may be mis-remembering).
I'm just trying to figure out a way that rather than a human who's being a sneaky red flag, he's really just a non-human green flag
