I honestly think that was the writers intent. Leave all the readers in the dark and confused til the end. The only thing you know is that Acchan and Ryouta will be together and something happened to make Akito not want to be called Acchan since you see it used without the retort of "it's Akito".
i think i enjoyed the manga as much as the interview of the real uke T-T that really is beautiful arggggg it's true love
