Caesar has always been portrayed as an extremely ruthless mafia boss, and Lee Won is well aware of the person he is and the toxic situation he's in. He simply chose to stay with him and accept him as he is (not that he's got much choice anyway ). But the story has never idealized them or passed off their relationship as "romantic", actually it's the opposite...
