Unelss the person used anesthesia or numb it, it won't hurt much but it mostly depends on the piercer's recommendation for the client. I also heard it depends on the type of pearl/piercing they plan on adding to the genitalia. Small ones doesn't hurt much after surgery. Deep piercings can cause severe implications though.
But who am I to imply real beaded dicks logic with BL fictional characters. Big beaded dicks are gorgeous and attractive for semes in BL HEHEHEHE for me at the very least.
