She obviously acts like a child and is very oblivious to everything around her and therefore he deemed it as wrong to view her romantically.
Additionally, he quickly noticed that something wasn't right, which is genuinely refreshing.
Most MLs would avoid Miesa, ridicule her, or touch her even if she acts like a child. Furthermore, most MLs wouldn't have picked up on anything and would have remained ignorant until the female lead FL speaks about it.
Idek how to feel about this... right now the author is clearly not romantizising it and even a toddler would know they're touched in the head,, but at the same time I'm not too sure since it's the beginning.
