No, the guy isn't intersex, he had hemorrhoids (he is cured now) it was also mentioned in the previous chapters that he has chronic enteritis

The pregnancy test thing was actually bought for their friend (she was scared that she got pregnant), but she returned it since she didn't need it anymore (probably got her period)

The dudes doing pregnancy test was to "confirm" that the guy is unable to get pregnant even though both knew that already. The black haired one (sorry forgot his name) actually hoped that there was a chance for Kashiwase to get pregnant since he uh wanted a reason for them to stay together and was just being sad that gay people can't have children in general
