I’ve been looking for a manga, i can’t remember it being yaoi or shounen ai... the setting is rather old, not modern world and with youkai/demons. They wore old Japanese clothing too.
It’s about a young man (looked like the bottom) who has some kind of curse/demon inside his body, which is passed down the generations I think (the curse was put inside the body with some kind of ritual, where they pierced his foot with a nail or so??). The curse spreads from his foot I think, throughout his entire body and it causes him tremendous pain. There were doctors who where able to manipulate the curse inside the young man using some kind of water bending (idk what to call it hi avatar fans)
