As a person who struggles with diagnosed mental illnesses, this could be a possible indicator that Chang-gyeon (emperor/ML) has a manic disorder. We can clearly see that in this chapter he is in the state of mania and does seem like he's lost his mind, but it could also have been triggered by his servant this morning, hence leading to a manic episode.
Chang-gyeon has his moments where he is seen in a state of mania, euphoria and rage. He is a man who experiences delusions, false beliefs and refuses to acknowledge the truth that's already in front of his face. This could all stem from a traumatic event that could've happened while Sam-myeong (doctor/MC) was away, which lead to Chang-gyeon's mania. Sam-myeong was his safe space, and what could have happened while Sam-myeong was gone, must've messed up Chang-gyeon in such a way for him to turn out to be a tryant and a lunatic.
I hope we get to see Chang-gyeon's past, or see if he's actually just clinically insane. I hope we get to see more plot, for this will be a long story of perseverance, as stated in the first episode.
when the king (ml) was waving goodbye to mc, it actually kind of made him look — in the least bit — humane. he looked like an actual man who hadn’t done anything wrong to mc, yet it also looked unsettling in a way. it felt like an admonition, a warning just from his smile.
