Some people can be born with both dick and vagina. Usually one of them will be cut out or something when the baby is young, because it's less risky that way, but sometimes they don't get cut out so the person can choose wich gentler they are. Idk if the manga has this, but this is an actual thing.

lol If you didn't know, some people can be born with both a vagina and penis. Usually, one of them is removed a little after they're born, but in some cases, they keep both. I don't know if this story is supposed to be like that, but I'm guessing it does. A lot of people don't know this, so it's okay that you didn't know. :)
