But it's close to impossible to be a true emperor and a doctor at the same time. As emperess, one must be constantly on alert or doing some political deals with the nobles and stuff. Being a doctor would only scully her imagine of empress. Moreover, a doctor must always be on call and always there for their patients, (you saw how quickly Elise had to act right?) what if she's treating a patient then was called to present herself as queen? If you reread the chapters before, you will notice that they have never called each other 'emperor' and 'empress' the whole time... I'm pretty sure it's acting so they can catch the idiot (you know whom I'm talking about) behind this plot quickly.
