If you're comfortable with ur deadname put it under the chosen name in small letters and inside brackets with "aka" like,
[CHOSEN NAME]
(aka [deadname])
If a teacher asks u, tell them that most people call u by that name (chosen name).
If you're not comfortable with ur deadname, just use the chosen name.
