I mean, the gigantic penis part yeah, but the milk part? It DOES work like that, its also not TOO painful because unlike piercings, the milk channel already exists, even in men. Hope this helps (=・ω・=)

(Sorry for the long reply !) It doesn't work like this. When biological males go through the male puberty, the testosterone stop their breast from developing, leaving undeveloped milk ducts. I don't believe you can just inject milk directly in the nipple and suddenly you're lactating as a male, it doesn't work like that and can actually cause infection. It's a needle that's going through the nipple, even if it's through that tiny hole, it still gonna hurt like a bitch, saying as someone who has his nipples pierced. It'd be a different story if it was a trans woman that started taking estrogen, then her milk ducts will start to develop and are fully able to lactate. However, some very few men are able to lactate while being biologically male and even then it's never natural and a big red flag that needs to get checked out since it's likely that it could be from cancer. A man won't lactate naturally in a non-harmful way unless he's taking estrogen and develops breast tissue (which takes months to years) or he's a transmale with no top surgery. This is just fetishized content, lovely, lol.
