Is called "fundoshi" and is the japanese traditional male underwear, it was used by all classes of people and from all ages, also is not from edo period, it was used through all japan history until the invasion of americans during WW2 when japanese were finally introduced to more comfortable undies.
The "baldness" is called chonmage and it was shaped like that so that samurai's hair could hold their helmet in place. Since samurai were so popular a lot of guys from all ages started to used it as a trend so the shogun of that time (Ieyasu) sent order for all the man to shave not only the head but also the face. Honestly is quite the turn off but I would rather see this than the dirty unwashed hair from European people of that same era
