Mikado yakuza dude has no love for the sub because he's a half-measure. He keeps on receiving his needs as a sub but hasn't quite done much for the dom. That's why Mikado and Kana don't have much respect for Sumio. He's a doctor who hasn't truly given his sub characteristics any chance to prosper. He's a sub who doesn't know what he wants because, deep down, he just hates his sub nature when, in the context of this world, there's no real reason for that.
In that sense, I do get the dislike for Sumio. 4 chapters in and he's just been a doormat. Receiving stuff handed to him. He hasn't really done anything to get himself healthy. Just coz he's a sub, that doesn't mean he shouldn't be active to get what he needs. I hope we get to see some character development soon.
Coz I can't help but sympathize more with Miho. It must be the introvert in me. True, Hiroya's outgoing and pushy character resulted in their friendship. But, I'm still no fan at how he handles himself in regards to Miho. He just tends to do what he wants. Case in point, Miho knew him enough back in high school that he knew, HE KNEW, that the kiss was done on impulse.
Fast forward to the reunion. Miho may still have feelings for Hiroya but overall, he went there with thoughts of friendship. After reading this chapter, we get to see Hiroya's whimsy yet again. He knows he can get away with it when he pushes. And he does. And, again, he acts on his impulses on someone who's deeply introverted and serious to a fault.
Can't help but sigh at them. For all the complications, I hope they talk. One just dives into escapism while the other just goes with his feelings. Hiroya never even thought of sitting down with his very serious friend and explain why he does what he does?

TALK DAMN IT.
