It isn’t obvious. There’s no reason for him to suspect for even a second that the MC is an omega. He’s known him all of his life, and he has no omega traits. Why would he think he’s talking about himself? It’s easy to say that as the reader, but it makes literally no sense for him to think that he’s pregnant.

It’s a BL manga. The seme played in the park with the twins during his childhood. Ofc, he developed a crush on one of them. I think he got the wrong name of his crush lol. He asked an adult at that time and I guess the misunderstanding started there. They all grew up and I remember one twin brother acted all kind and innocent to fool the seme. The uke was actually the other twin brother instead. The uke and seme relationship wasn’t that good at first. But seme soon realized that uke was his crush during childhood.
