Is it just so they can villainize her to an extreme extent?” and there might be people who wondered the same thing as me. I fugured that the answer is that mages in fantasy worlds mostly have high reputations and status because they are considered a very useful asset of the state/empire. In other words, most of them are high-profiled individuals and are difficult to control while doctors existing in fantasy worlds where magic is present might be nobodies even considering their skills since one can easily use healing magic. This is why doctors are easier to control and thus the way for mc’s mother to conceal his real secondary gender is a painful one. It actually make sense.
