Well actually, it's a lot more common than you think in history. Remember a lot of these marriages were arranged and it wasn't me from love. Rereading the story, I saw how her downfall began. She didn't have anyone on her side to REALLY help her, scold her, lead her in the right direction. All the people just wanted to be in her good grace. If he really loved her, he would get her an education when she stepped there, but he didn't want, he even said he didn't want her to change, but its obviously she had too.

Well human body is 3d so that doesn't happen really, and this is 2d so first there's this issue of dimension and ofc they have organs butttt while reading something like this you don't wanna see their intestines and kidneys , so they can't draw it , I'm sure there's whole another genre for that. And ofc the scene doesn't go so smoothly and specifically no lubes, lord nobut again we're like a hours of process in minutes so they don't wanna break flow. And yeah that doesn't reach that far to be trying to come out of stomach, not usually .
But yeah I do agree it's so unrealistic and take it too far at times. But it's just a fiction.
