Ehh...? Are they step sibling if I'm not wrong when i watched the anime they only show they are sibling so i don't know... I kinda disappoint but not entirely because the anime is really Good if I'm not mistaken the brother/MC doesn't show romantic affection to his sister but the sister show romantic affection like jealousy so It's a relief if they are not real sibling ..... Beyond anime I don't know what happen next, I tried to read manga but the manga is not complete translated

tatsuya has huge op powers and his mom and aunt were concerned that he could/would destroy the world, not unfeasable for him, so they created a child with the height of intelligence and abilities. they used the moms embryo and 'designed' the child to become the limiter/seal of tatsuyas powers. hence miyuki being made just for tatsuya, while also being his 'sibling'. kinda. this is why she is also op talented and why she can seal his powers.
on a side note her sealing his powers is why she struggles to control her own, freezing her surroundings when upset. her magic is busy sealing his abilities, so when she breaks the seal her control is much better. this is also why she struggles with how much power to use. we dont see the side effects much as she has so much power she doesnt run out after a few spells and she seems to err on the side of caution, like when she killed the terrorists, though tatsuya saved them. we see her improvement later when she managed to only stop the ghost walker, not kill him. also its not like she used a stupid amount of power for the spells which is also why its not obvious she has this problem.
this isnt well known as even miyuki and, i think, tatsuya are unaware of this. towards the end of the novel series one of them, pretty sure its miyuki, gets adopted by their aunt and declared her heir, thus becoming the next head of the clan. their 'real' relationship gets revealed to them their link to the yotsubas gets revealed to everyone and their engagement to each other is announced. the rest of the world just thinks that they were cousins/people from the same clan, but were incognito as siblings, so dont bat an eyelash. miyuki would, probably, be thrilled at the engagement and as she is the only person tatsuya can love he is fine with their engagement as well. i think it ends soon after that so we dont learn if they have kids or if its a platonic type of relationship or if they live seperate to the clan either. i know the experiment he went through removed his extreme emotions and that includes extreme lust but he does still feel lust. apparently its just kinda muted or its easy for him to focus on other things. kinda just see it as him having a low sex drive to be honest as im not sure i fully understand it. hope this helps. Actually it's Tatsuya who gets adopted by Maya, their aunt. So he became Yotsuba Tatsuya altho still preferred to use Shiba.
