If you read the unofficial translation, it seems that the sister is the one who went to the hospital but if u read the official one... Its much more confusing and ambiguous but base on my understanding on the official translation....

well, in my understanding, shougo met up with the sister (at the hospital), hence the strong scent (he said that the sister liked to wear strong perfume, if he was talking about the babysitter, that wouldn’t make sense cus how would he know their preferences). as for the child, either shougo or akira probably asked someone to look after him for a short time while they were both at work, or maybe (shougo) left him to the babysitter at the sister’s house and went to pick him up when he dropped the sister off.

granny being dead is a good catalyst for the story to move forward, the reason why dan is there in the first place is because of his grandma. Jk has him on hold because dan mainly needs money (thats why he kinda ask dan if the money that he gives is not enough, idr).
