i remember one part of the manga and the author might be Naono Bohra ... is mc is a doctor or surgeon and the seme? is the son of a gang leader? in the scene i remember the mc pulls a scalpel out and threatens his attacker. that's all btw they live in the same apartment building..

I took bits and pieces from the manga you're searching for (the younger seme with messy hair) and the closest one I could remember was

(He has messy hair and is very puppy like and ex-lover constantly bothers the uke at his apartment but I do t think he skates or has dreadlocks...)
So maybe it's not the one but it's pretty similar (or maybe you're mixing up two mangas?)
