The doctor said that if anything happened to Guwon while he was in town, the Hounds might harm the townspeople. That's why the doctor went out of town with the teacher, because he knew Guwon would follow them to see if the teacher was okay (after all, everyone knows he's important to him). The doctor wanted to kill Guwon outside the town so the townspeople would be safe from Guwon and his pack.

I don't know what's true anymore. In chapter 0, Guwon kills a maggot that wanted to hurt the teacher. In chapter 24, we learn that it was actually the Teacher's father. It's obvious that Guwon killed him. But Guwon couldn't know if the father was useful or not, because he didn't have the opportunity to talk to him. Guwon didn't kill people until he found out if they could be useful to him - he talked to the Teacher first, even though he wanted to kill him. The father's age didn't matter either, the Doctor and he were middle-aged. Maybe the father was infected, but he hid it from the teacher so as not to scare him.
