This story broke me, it broke me so much that I can't put it into words to explain the impact it had. So I'm ignoring all reality and canon and manifesting an alternate universe where the doctor and MC find love (Fuck that bastard seme, he doesnt deserve to live in this alternate universe but he will, for "plot")

It's been awhile since I read this (I needed to recover emotionally) and maybe some things are wrong but this is my au:

-MC and fucktard did get together in high school but they didnt last, mc lost feelings and felt that fucktard was holding him back, fucktard still has feelings even after they separated (this will be important)
-MC gets to go to medical school and become a doctor, this is where he meets the doctor
-doctor is popular at the school whereas mc is not but they finally talk when mc has a particular book that the doctor needs (research material), so they decide to share it.
-they continue meeting up at the library to share the book, but once that's over the doctor musters up the courage to ask him out
-MC thinks it a friendly hangout and says yes, doctor thinks it's a date. They go on a date but the doctor soon realizes that mc thinks they're just hanging out, so at the end of the date the doctor confesses.
-mc is shocked (he likes the doctor too over the period of their library meet ups but has realized it yet) and the doctor declares that he'll seduce mc until he gets his answer.
- the seducing begins, it's very successful and they do get together in a wholesome confession
-marriage obvs
-the cancer thing will still happen to mc except this time he'll actually get the treatment and be cured.
-he retires from being a doctor, as the cancer has affected him physically, so he cant do the rigorous work anymore, and becomes a gardener.
-they live happily together with their dogs and cats and whatever strays they find (animal or plant). And possibly a child?

Now what about that scumbag? Well in this alternate universe the mc and doctors heaven or perfect universe is that assholes hell. He will experience the pain and suffering he inflicted on others, he will live through it with no way out. He'll have memories of his other life and remember how good he had it, he will still remember his guilt and mistreatment of mc but he cant do anything about it in this life, he wont be able to escape his punishment as he wont be able to atone for his sins and watch mc get treated right and be loved by a man that is loyal and devoted to him.
